Assessment Pattern
This module will be assessed through a 3000 word reflection exploring a case which they have managed. Students will need to explore their own development in enhancing their role within case management.

Module Aims
This module aims to develop the skills of case management to enable health and social care professionals to deliver improvements in caring for people with long term conditions.
Learning Outcomes
On successful completion of the module, the student will be able to:

Knowledge and Understanding
• demonstrate an advanced understanding of generalist and specialist assessment tools for case management e.g. single assessment process
• demonstrate an in-depth understanding of the role of targeting, commissioning and procurement for case management for fair access to services

Cognitive Skills
• critically appraise the role of the case manager
• critically evaluate risk assessment and risk analysis
• embrace the theoretical mechanisms which inform decision-making and it’s application to the context of case management

Practical Skills
• co-ordinate, organise and monitor complex care packages to support personalised care plans
• demonstrate competency in providing care to vulnerable people with long term conditions to enable and promote independence, dignity and choice to maximise independence
• evaluate the effectiveness of tools for client outcome measures
• evaluate service outcomes using effective data collection

Key Transferable Skills
• demonstrate awareness of the need for cross-disciplinary working
• demonstrate awareness of the legal and ethical considerations in case management

Leading and pro-actively managing complex care co-ordination

Methods of Teaching/Learning
The module will be facilitated using an enquiry based learning approach, with keynote lectures by relevant professionals and student-led seminars. This will develop the key skills of negotiation and cross-disciplinary working integral to the role of case management.
